All applications materials, including reference letters and transcripts, should be sent together as one complete packet. Incomplete packets will not be accepted. If we receive an incomplete application package, it may be returned to you for completion and resubmission, but you will be assessed a $25 administrative fee.

  1. Official University transcripts

    Send in a sealed envelope an official university/college transcript from each school attended.

    Degree: A minimum of a four-year baccalaureate degree (BS, BA). (International equivalency may be assessed with World Education Service).

    Prerequisite courses:

    • Biology
    • Chemistry
    • Physics
    • Statistics
    • Psychology
    • Algebra or higher math
    • Human anatomy
    • Human physiology

    Three semester hours or their equivalent are recommended for each course. If a combined human anatomy and physiology class is taken, six semester hours is recommended. It is further recommended that a gross human anatomy course with a dissection lab be taken, if available. A minimum grade of C is required for all prerequisite courses. For more information on the required prerequisite courses, please see our sample course descriptions.

    Recommended courses:

    • Kinesiology
    • Biomechanics
    • Material science
    • Public speaking
    • Business

    Recommended experience:

    Applications are looked upon more favorably when actual prosthetic and or orthotic experience has been attained. We suggest that applicants gain as much experience as they can prior to attending the program. Experience is recommended however there is no minimum time requirement to apply. Students can find a list of ABC certified practitioners and accredited laboratories at the American Board for Certification in Orthotics and Prosthetics website. It is recommended that students develop and continue an affiliation with a laboratory near them to enhance their educational experience throughout the program.

  2. Letters of Recommendation

    Two Letters of Recommendation are required from clinical professionals such as a certified O & P practitioner, physician, or allied health professional such as a physical therapist or occupational therapist. Forms are downloadable below.

  3. Written Essay

    Write an essay describing why you wish to enter the program and profession of O & P

  4. Northwestern Application

    An application for the certificate course is downloadable below as a PDF document. The application can be filled out using Adobe Acrobat (or typed on a typewriter), printed and mailed in with your application fee. Applications must be sent as a complete package, including your transcripts and recommendation letters. We do not accept faxed or handwritten applications.

Please note: Should you receive an acceptance package after applying to the certificate program, you will have 30 days to affirm your acceptance and provide a tuition deposit of $2,500. THIS TUITION DEPOSIT IS NONREFUNDABLE.
